Outreach Support for Adults with Learning Disabilities and Autism
Options for Life provides flexible, person-centred outreach support for adults with learning disabilities, autism and additional support needs across Sandwell.
Our staff can meet individuals at home or at an agreed location and provide support to access the community, develop independence, build confidence and take part in activities that are important to them.
Every person's outreach support is different. Activities and goals are agreed around the individual's interests, abilities, support needs and desired outcomes.

Person-Centred Support
Every participant has an individual support plan developed with them and, where appropriate, their family/carers and relevant professionals.
We can also provide regular communication, maintain relevant support records and work alongside other professionals involved in the person's care and support.
Options for Life Outreach
Options for Life supports adults with learning disabilities and/or autism by providing outreach provisions, hub-based activities and drop-in sessions increasing their self-confidence, developing communication and social skills leading to independence.
