Outreach

Our outreach service was developed in 2020 in response to the COVID-19 pandemic and has proved to be a popular service that we will continue to progress and develop.

 

As with all of our services outreach activities are focussed on achieving positive outcomes for people. 

 

What’s included:

  • In depth support plan co-designed by parents/carers, professionals and the participant

  • Person Centred Learning Plans - to support life skills, independence and social skills

  • Daily/weekly communication Logs - including seizure charts, medication paperwork etc

  • Support for parents/carers if required for professional meetings

  • Opportunity to join a parent/carer forum, run by parents and carers

  • Monthly communication updates from assigned link workers

 

Personalised support with:

  • Developing and maintaining life skills

  • Healthy lifestyles

  • Voluntary or work placements

  • Travel training

  • Social life and leisure activities

Options for Life Outreach

Options for Life supports adults with learning disabilities and/or autism by providing outreach provisions, hub-based activities and drop-in sessions increasing their self-confidence, developing communication and social skills leading to independence.
